Apply under the name you plan to use professionally as a Genetic Counselor. The Genetic Counselor Licensure Program asks about other names used and will be able to recognize previous names used for matching purposes.
On the Genetic Counselor License application form, a question is asked about whether the applicant is a US citizen. Are individuals who are in the United States working under a Visa or Green Card able to be licensed?
Under California law, individuals authorized to work in the US with a H1-b or other Visa or Green Card may be licensed if they have a Social Security number.
Therefore a non-US citizen must submit a copy of their status and a copy of their Social Security card.
