FFY11 Network Annual Progress Report

The FFY11 Annual Progress Report (APR) documents are below. The Annual Progress Report Form and the Annual Progress Report Narrative are cumulative and intended for reporting on your Scope of Work activities encompassing the entire Federal Fiscal Year (October 1, 2010 – September 30, 2011). The Activity Tracking Form and supplemental SurveyMonkey that are due in conjunction with the APR are not cumulative and will only include activities conducted from April 1, 2011-September 30, 2011.
Webinar trainings to review how to submit the APR are scheduled for September 6, 2011 from 9:30am-11:00am and September 14, 2011 from 1:30pm-3:00pm. Registration is required and can be done here: https://www.surveymonkey.com/s/CJ67Q72.

The Annual Progress Report documents and online SurveyMonkey are due to be completed and submitted by Monday, October 17, 2011. Please contact your assigned Network Program Manager with any questions or to request an extension on your APR.
