Common Ground Activities in CDPH

The California Department of Public Health (CDPH) is the project lead for the Common Ground grant which was awarded to the California Health and Human Services Agency (CHHSA) in December 2006. This three year planning and training grant has two components: a national collaborative project and a local project

National Collaborative

CDPH has been an active member of the Chronic Disease national collaborative to conduct business process analysis, redesign, and requirements definitions for a subset of chronic disease activities in public health. California Project Activities

CDPH has used the resources provided by the Common Ground grant to improve business processes within the department and with other departments in CHHSA. The vision presented in the grant application was to create a portal for chronic disease data that (1) provides de-identified geographically-referenced data for program assessment, disease pattern analysis, and targeted resource planning; (2) accesses and combines information from a range of data sources; (3) facilitates improvements in health status and outcomes through both primary and secondary prevention; and (4) complies with Public Health Information Network (PHIN), National Electronic Disease Surveillance System (NEDSS) and Content Standard for Digital Geospatial Metadata/International Organization for Standardization 19115 (CSDGM/ISO 19115) geospatial data standards. To this end, CDPH has focused on business processes related to the presentation of information on the CDPH website and on core centralized GIS capabilities.
