This checklist presents an overview of required materials for device certification. Please refer to the statute and regulations for detailed information on certification requirements.
- Application fee of $1400 (non refundable)
- Completed application form
- Contaminant reduction test reports. Lab must be California approved; testing must be according to California-approved protocols.
- Extraction test report for entire system. Lab must be California approved; testing must be according to California-approved protocols.
- Device or system label with required information.
- Performance data sheet with required information.
- Promotional materials must be consistent with certified claims. These are reviewed during the application process.
- Engineering drawings and parts list for system.
