                          Water Treatment Device Certification:  Checklist of Required Materials

                          This checklist presents an overview of required materials for device certification. Please refer to the statute and regulations for detailed information on certification requirements.

                          1. Application fee of $1400 (non refundable)
                          2. Completed application form
                          3. Contaminant reduction test reports. Lab must be California approved; testing must be according to California-approved protocols.
                          4. Extraction test report for entire system. Lab must be California approved; testing must be according to California-approved protocols.
                          5. Device or system label with required information.
                          6. Performance data sheet with required information.
                          7. Promotional materials must be consistent with certified claims. These are reviewed during the application process.
                          8. Engineering drawings and parts list for system.
