Please fill out form LAB163, obtained through the link below, as completely as possible.

The form can be e-mailed, faxed or mailed to Laboratory Field Services.

All complaints are confidential. 

Complaints are also taken over the telephone. 

When making a complaint, please include as much specific information as possible, so that Laboratory Field Services can gather all needed information.
