Welcome to the State of California 

The California Department of Public Health (CDPH) Healthcare Associated Infections Program (HAI) would like to provide more meaningful infection control support and education to California’s long-term care facilities (LTCF). We would like to understand from LTCF Infection Preventionists and administrators what the needs and challenges are when implementing infection control measures. Your facility can provide this information by completing a survey tool that may be accessed at:

You may wish to download and print the survey to complete it on paper prior to entering your answers in the survey monkey site. If you choose to have more than one staff member contribute to the answers, please ensure that your designated Infection Preventionist is part of the team completing the survey. The survey should take approximately 30 - 45 minutes to complete. The HAI program will collect and summarize these data, aggregated from all LTCFs that participate in the survey. No LTCF-specific information will be shared or released if provided; only aggregate data will be reported.

With the information you provide, we hope to better understand and serve you in a consultative capacity and to support educational programs to meet LTCF infection control needs. The survey results will be summarized and LTCFs will be notified when they are available at:
