The Alameda and Sacramento forms must be used in combination with the 24- Hour Perinatal Dietary Recall, Prenatal Nutrition Assessment, or Perinatal Food Group Recall, as well as the appropriate weight grid for the appropriate prenatal weight range. These forms are available at http://www.cdph.ca.gov/pubsforms/forms/Pages/MaternalandChildHealth.aspx
These must be used with the Perinatal Food Frequency questionnaire or another approved nutrition assessment (see above), the weight grid, and a separate Individualized Care Plan.
