Purpose
To provide tools and guidance necessary to evaluate the BBC project
Description
The evaluation toolkit includes various tools to monitor progress towards implementing model hospital breastfeeding policies and practices. The tools also facilitate the Plan, Do, Check and Act (PDCA) performance systems analyses technique (quality improvement) .
Figure: illustrates the implementation and evaluation of the BBC in relation to the PDCA model .
These tools include:

California Model Hospital Breastfeeding Policies Self-Appraisal Questionnaire (MHBFQ) (Word)
Quality Improvement Action Plan (Word)
Hospital Network Meeting Evaluation Form (Word)
Train-the-Trainer Workshop (16 Hours)
BBC Hospital Breastfeeding Data Collection
Together, these tools should be utilized throughout BBC implementation to track the hospitals’ progress.
