RHB - Mammography Facility Certification

1.         Where can I find out how to apply for a California mammography facility certificate?

2.         Do I still need a California Facility Accreditation Certificate for each of my mammography machines now that the ACR is my accrediting body?

Yes. You need to have been issued either a California Interim Facility Accreditation Certificate or a California Facility Accreditation Certificate before you can legally perform mammography.

3.         Once we send in our application, can we start doing mammography?

No. You cannot legally begin doing mammography until you have received a California Interim Facility Accreditation Certificate for the machine you will be using.

4.         Do we need to notify you when we get new staff, repair or modify our mammography equipment, or move our office?

Yes. The administrator, owner of facility or their designee needs to send us a written signed and dated notice within thirty (30) days of any of the following events: 

  • Change in the information submitted in your Application; 
  • Change of your accrediting body; 
  • Change in your FDA certification status; 
  • Change in your facility personnel, interpreting physicians, mammographic technologists, or medical physicists; 
  • Change in the location of any mammography radiation machine within the facility; 
  • Change in your facility’s name or owner;
  • Disassembly and reassembly of any radiation machine or processing equipment you use for mammography;
  • Change in your accreditation status;
  • Change of your facility's contact person.

5.         Can I just send you a copy of my ACR and FDA certificates to be able to receive a California Mammography Facility Certification Certificate for my new mammography machine?

No. You will need to finish the application process before we can issue you a California Facility Accreditation Certificate for your new mammography machine. 6.         If we just had our MQSA inspection performed by the State, will we automatically be issued a California Mammography Facility Certificate.

No. You will need to finish the application process before we can issue you a California Facility Accreditation Certificate for your new mammography machine. 7.         Do my MQSA fees include the fee for registering my mammography machine?

No. The MQSA program is Federal while the mammography machine registration is a State program.
